Y1 - 4 - Flu Vaccinations
Dear Parent/Carer
We are in your child’s school delivering the nasal flu vaccination to those whom we have consent for. If there are any changes in your child’s health i.e. they are unwell or have had an increase in their asthma medication or worsening symptoms or if they have received the vaccination at your GP’s surgery since completing the form, then please write to the school by 08.45 on the morning of the vaccinations, Wednesday 22nd November 2017, informing us of the changes.
